1. REGISTER TODAY! Day of camp walk-in rates are $175 for 3rd-12th grade and $75 for 1st-2nd grade. All camps will be held at the Arrowhead Center in Elk City. There is a map of the location listed below. 

 

2. Every camper must print out and bring the registration form below filled out completely the first day of camp with a copy of your medical insurance card in order to participate. There will be an on-site registered nurse for any medical needs.

3. Each camper will receive a t-shirt, wristband, group photo and at the end of the camp prizes will be given to top performers in selected categories.

 

4. All campers must come dressed and ready to play basketball. Water will be available, but you are welcome to bring your own if you choose. There will be plenty of basketballs, so there's no need to bring your own. We are not responsible for any lost or stolen personal items.

 

5. * All payments must be paid in full by July 1st before reserving your spot for camp. Payments are accepted via Cash App, Venmo (@cadedavisbasketball) or cash or check only. Please make checks payable to Cade Davis LLC. and mail to:
